A Clinical Observation of the Intussusception in Infants and Children



Abstract
A clinical evaluation has been performed on 197 cases arisen from 184 patients who were admitted to the Soonchunhyang medical college hospital during the period of four and half years, from Jul. 1979 to Dec. 1983.
The following results were obtained;
1) 85.4% of the patients were under 1 year of age, and the highest incidence was found from the fifth to the ninth month of life.
2) The ratio of male to female was 1.6 : 1.
3) There were no great differences in the incidence among the four seasons, but we could find out a little higher incidence in summer(31.5%).
4) The etiology of intussusception remains obscure in the vast majority of instances, and 89.8% of all cases reported were of idiopathic variety. Rest of them had associated factors; such as the mesenteric lymphoid hyperplasia(6.7%), enterocolitis(2.0%) and mobile cecum (1.5%) and so forth.
5) A significant number of the patients(77.7%) were visited our hospital within 24 hours from the onset of the symptoms.
6) Four cardinal clinical manifestations were vomiting(84.3%), abdominal pain(78.1%), bloody mucous stool(74. 1%), and palpable.. abdominal mass(63.5%).
7) In the study of the plain abdominal films, 80.2% of patients revealed the positive signs of the intestinal obstruction, and the commonest finding among them was gaseous distention of the small bowel(62.9%).
8) The commonest anatomic type was ileocolic type(40. 9%) and ileocecal type(22. 6%) was the second most one.
9) Barium reduction were tried in 194 cases. Among them, 101 cases were effectively reduced, and the success rate was 52.1%. 93 cases out of 96 cases who had been operated had been performed the manual reduction, and remained 3 cases had been spontaneously reduced at celiotomy.
10) The success rates of barium reduction according to the duration from onset to admi-ssion were 60.7% within 12 hours, 50.0% within 24 hours, and 21.4% over 48 hours from onset of symptoms. And this result revealed close relationship between the success rate and
the duration of the symptom. The longer the duration, the less the success rate.
11) Postoperative complications occurred 16.7% of all cases reported.
12) Overall recurrence rate was 7. 0%, and the rate following barium reduction was 11. 9 and was 1. 0% following operation.
13) The mortality rate was 0.5%.
14) The number of the average admission day was 3.2 days after barium reduction and 7 days after operation.
